How do I get setup as a TurnStone Freight Carrier?
We take pride in building a network of motivated and reliable freight carriers that meet the highest safety standards, providing the best service for our customers.
The following documents are required to have on file from your company:
- Cargo & Auto Liability Insurance Certificates – The certificate holder must be listed as TurnStone Logistics LLC, 401 East Sonterra Boulevard, Suite 375, San Antonio, Texas 78258. NOTE: A faxed copy of your insurance certificate can be accepted as temporary proof of insurance for set up while your insurance company prepares the certificate with TurnStone Logistics LLC to be listed.
- FMCSA Certificate of Authority Letter
- W-9 Form – The information provided in your w-9 must match your IRS filings.
- Carrier Profile – This helps us find you more freight for your trucks. If you use a factoring company, their name and address must be listed in the area provided AND you will need to include the “Notice of Assignment” to the factoring company.
- TurnStone Logistics Broker-Carrier Agreement – Each page must be initialed in the bottom right of the page and the last page must be signed and dated.
Approved carriers must meet the following requirements:
- Valid MC# and associated Authority to Operate. Intra-state carriers may provide a DOT# only.
- Valid insurance – $1 million in Auto Liability, $100,000 in Cargo, and Worker’s Compensation in amounts required by your state of incorporation (but not less than $500,000). Insurance certificate must be received directly from your insurance agent.
- Satifactory Safety Rating – If you have a conditional safety rating you may accepted on a case by case basis by providing a Letter of Safety stating why you have a conditional rating and what actions are being taken to improve your safety rating.
- Insurance company must have an A.M. Best rating of A- or better.
- Must have an active authority for at least 90 days.
- Must have three(3) broker references.
